In order to better prepare for our workshop, please leave a detail message about what concerns you would like to address during our session and any information you believe we need to know.

Parent / Guardian Agreement *

Educator will remain respectful and courteous at all times.
Any session cancellation must be done at least 24 hours prior to scheduled time. Sessions that are cancelled less than 24 hours prior to the scheduled time will incur a 15% fee.

Parent / Guardian must agree to adhere to the expectations set forth by Cultural Ambassadors for Youths, whether these are discussed with you orally or in writing.
